Chronic Disease Management Clinics
We currently operate a number of clinics for routine check-ups for patients suffering from
a range of Chronic Diseases. This includes Asthma, Chronic Obstructive Pulmonary Disease (COPD), Cardiovascular Disease (CVD), Diabetes, Hypertension, and Chronic Disease Management (CDM). If you suffer from any of these conditions, you will automatically be invited for routine assessment(s). These clinics can be booked on a day of your choice, providing the appropriate clinician is available that day.

Keep Well Clinic
The Keep Well Clinic is aimed at improving the health of the community. This clinic is offered to patients between the ages of 40-64 who meet a certain criteria. These sessions are completed by our Practice Nurse and involve the follow:
- Checking your Cholesterol
- Measuring you Weight
- Taking your Blood Pressure
- Talking about keeping a healthy heart, lifestyle and anything else you may be worried about.

Cervical Smears
The practice nurse performs routine smears. Appointments can be made at reception in advance and patients are recalled automatically every three years.
First contact physiotherapy Service
We have a first contact physiotherapy service who work with the practice. They offer face to face appointments out with the surgery which can be booked up to 1 week in advance. This service is for any muscle, back or joint pain you may have. You do not need a referral for first contact physiotherapy service. If you think you would benefit from first contact physiotherapy please speak to our reception team who will be happy to book you in for an appointment.
PALMS (Patient Assessment and Liaison Mental Health Service)
This service is for anyone aged 16-64 (not in school education) who is experiencing mental health difficulties such as low mood, anxiety, depression, self esteem issues, distressing life events or other similar difficulties. Exclusions for this service would be those who already see a mental health or drug & alcohol professional, such as those at Alloway or Constitution House. If you think this service would be beneficial to you, please speak to our reception team who will be happy to book you a face to face appointment.
SOS (Sources of Support)
The SOS service is for patients aged 16 or over whose mental health and wellbeing are affected by social and economic issues such as debt, housing, benefit support or similar. The link workers negotiate, liaise, advocate and communicate on the patient's behalf at the same time as they help to build the capacity and resilience of the patient to deal with issues themselves. These appointments will be face to face or phone and not necessarily always in practice.
Listening Service
The Listening Service is for patients who are looking to talk with someone, particularly during stressful events at home or work, or after a bereavement or loss. Patients will get the chance to tell their story uninterrupted and in a judgement-free zone to help them sort through their emotions. Appointments for the Listening Service are face to face, in the practice.
